On Sunday 23 Dec 2012 at 6 pm : we received 8 cases to the field hospital inside besieged Homs districts , these infected persons had inhaled unknown gazes and they were suffering from headaches, nausea and shortness of breath in addition to generalized body pain and redness of the eyes with epiphora , Some cases in which inhaled large amounts of gas, the infected person was unconscious and suffering from a variety of neurological disorders .
We Had to deal with these first cases as cases subjected to inhalation of large quantities of tear gas in closed places where there is no air enough .
Immediately after that, we received many cases with the diversity of symptoms, in addition to the previous symptoms, pain episodes with agitation and unconscious and muscle spasms in some cases .
Patients who have had precedents chest diseases such as allergic asthma was the case they have much tougher and shortness of breath and it was very clear.
And the psychological impact was seemed on the patients more than disease condition .
The fundamental problem that everyone is having severe stenosis in the pupil , we called it ( pin pupil ) and this is what we are unable to explain, which made us think that it might used phosphorus compounds in this gas .
The Summary of clinical examination, signs and symptoms for most cases:
1/ The majority of the patients had good consciousness.
2/Most of the patients were suffering from
A- The eyes symptoms : : Redness with pain and heartburn , Photosensitivity , Severe stenosis pupils and Blurred vision
B- Digestive symptoms : Nausea , Sometimes vomiting , Bouts of Intestinal and stomachic convulsions
C- Thoracic symptoms : Difficulty breathing is associated with hypoxia
D- Neurological symptoms : Headache , Severe psychological tension , Sense of imbalance , Some patients with disorders of consciousness varying intensity .
3/ Clinical signs :
A- Situration within normal limits.
B- pressure and pulse check within normal limits .
C- Chest examination: Normal breathing sound , some patients have laryngeal stridor
D- Neurological examination in patients with non unconscious: Heading of time and place, and they don’t have heightened or absence of reflexes.
The Treatment : all affected cases have been treated through put oxygen, nebulization sessions, give intravenous hydrocortisone , using eyes kind drops , washing eyes with physiological balserom .
For patients with irritable, were given Penzodiazepam and Midazolam compounds carefully , and some cases were given paracetamol painkillers , others were given a systemic anti-vomiting , and vital signs were monitored in all patients.
Most of the patients improved their condition gradually and was graduated from the hospital except those cases of loss of consciousness, which we expect to be caused by inhalation of large amounts of gas, and there are people with unconscious condition later has improved.
At the next morning we reviewed many of the cases that have been graduated and proportional healed , and they are now suffering from some headaches and nausea with some muscle spasms and a sense of shortness of breath . In addition to treatment, there was also a concentration on psychosocial support.
Injuries Statistics: 6 cases of death , more than 65 affected cases and 10 of them in critical situations.
